1st appointment Information

On your first visit: 

• Complete the intake forms under (REGISTER AS RETURN/NEW PATIENT) prior to your first appointment!
• Arrive 30 minutes prior to your schedule appointment time. 
• Bring your up to date insurance card and legal photo identification.
• Bring physical therapy prescription. 
• Bring relevant X-rays, MRI's, CT scans, and M.D. reports. 
• Wear shoes and clothes that you can move around in.
