Mission Statement
Ho Rehabilitation Center was founded in 1983 with the vision of providing quality health care to patients of all ages who are in need of physical therapy. For more than 30 years, Dr. Ho and his staff have achieved a high level of patient care through the integration of clinical expertise, current research and continuing education. Ho Rehab Center is committed to the pursuit of excellence through the practice of art and science in physical therapy.

Our philosophy at Ho Rehab Center is to provide individualized and holistic treatment based on the unique needs and goals of each person. An appropriate treatment plan is based on a thorough neuromusculoskeletal evaluation. Our goal is to relieve pain and facilitate healing, improve functional status, provide patient education for prevention and promote wellness.

Our Staff

Dr. Larry Ho, MS, DPT, OCS
founded Ho Rehab Center, Inc., in 1983. He graduated from National Taiwan University in 1972 with a B.S. in Physical Therapy. In 1974, he obtained his M.S. from the Medical College of Virginia and subsequently acquired his Doctor of Physical Therapy degree from USC in 1997. Dr. Ho has published numerous research articles in various scientific journals, translated the Manual Therapy book by Professor Freddy Kaltenborn into Chinese and for the past 20 years, has taught at USC in the Department of Biokinesiology and Physical Therapy as Adjunct Associate Professor. Dr. Ho travels extensively to China and Taiwan conducting lectures and workshops.

Dr. Nicole Tapking, DPT, CSCS
graduated from California Polytechnic State University, San Luis Obispo with a Bachelor of Science in Kinesiology in 2000. Her desire to pursue a career in the medical profession and her experience as a physical therapy aide led her to the University of Southern California, where she received her Doctorate of Physical Therapy in 2004. Shortly after graduating, she became a Certified Strength and Conditioning Specialist to further her knowledge in exercise prescription. Dr. Tapking has received extensive training in treating patients with oncology-related conditions. In addition, she enjoys working with our pediatric population, helping them return to normal function.

Dr. Aimi Mizumoto, DPT
received her Doctorate of Physical Therapy from University of Southern California in May of 2014. She comes from a broad athletic background, which fuels her interest in orthopedic and sports-related physical therapy. She has extensive experience in treating those with neck, shoulder, and back pain and also specializes in treating people with a variety of neurological diagnoses including Parkinson's Disease and Cerebral Palsy. Additionally, she takes this passion to her involvement with the community and has become an active volunteer/assistant coach with the Special Olympics.

Dr. Allison Kirksey Lombardo, DPT, ATC
is from Norcross, Georgia and got her Bachelor of Science in Exercise and Sports Science from the University of Georgia. She attended physical therapy school in Savannah, Georgia at Armstrong Atlantic State University. Allison moved out to Los Angeles and completed the Orthopedic Residency at University of Southern California in Fall 2014. She is also a certified athletic trainer and enjoys working with an athletic and competitive population with a variety of orthopedic conditions. She has extensive experience and expertise in treating patients with hip, knee, shoulder, and spinal disorders.

Sheila Richards, PTA, CPT
has been on staff with Ho Rehab Center since 1985. She graduated with a B.S. in Health Education from CSUN and continued on to become a licensed Physical Therapy Assistant. In 2004 she became a Certified Personal Trainer through the National Academy of Sports Medicine. In addition to patient care, Sheila is the office manager & leads group exercise classes certified by the Arthritis Foundation.

The Facility

Ho Rehab Center, Inc, was established in 1983 in Tarzana, CA, serving the outpatient physical therapy and rehabilitation needs of the San Fernando Valley. We have 7 private treatment rooms with a fully equipped gym. We specialize in sports, orthopedic and neurological movement disorders of the spine and extremities, serving both the pediatric and adult populations.

Ho Rehab Center is equipped with state-of-the-art rehabilitation equipment to not only facilitate, but exceed our patients' and clients' expectations and goals for physical therapy and fitness. We have an eclectic array of equipment to serve the diverse needs and capabilities of our patients and clients. Our friendly ambiance and humorous staff will help you laugh your way to recovery!

General Information

Insurance

  • Is Ho Rehab Center a Medicare Provider?
    Yes, we are a Medicare Provider. After the Medicare annual deductible (may vary each year) is met, Medicare pays 80% of reasonable and customary charges (80% of the allowed amount). If you have Medicare supplemental/secondary, we will be happy to bill them for the difference if they do not cross over from Medicare. However, this is never a guarantee of payment. If you do not have Medicare supplemental coverage, you are responsible for the remaining balance. An initial prescription for physical therapy is valid for the first 90 days, commencing with the initial physical therapy evaluation. Thereafter, Medicare requires a current prescription from the patient's M.D. every 90 days.   • Will Ho Rehab Center bill my private insurance?
    Yes, Ho Rehab Center will bill private insurance. Please note that all patients are responsible for their deductibles and co-pays. It is your responsibility to know what your physical therapy benefits are. Please visit our Links section to see if Ho Rehab is a provider for your insurance carrier. If you are still unclear about your benefits, our office staff will be happy to contact your insurance company and check your physical therapy benefits prior to your first visit.
  • Does Ho Rehab Center take HMOs?
    No, we only accept PPOs and POS.
  • Is Ho Rehab Center a PPO Provider for my insurance?
    Ho Rehab is an in-network provider for many insurance companies. Please contact your insurance company for participating provider information. If Ho Rehab Center is not a participating provider for your insurance, your benefits may still cover your outpatient physical therapy at Ho Rehab Center at a non-participating provider rate.
  • Is there a limit on the number of physical therapy visits I am allowed?
    Some insurance companies limit the number of outpatient physical therapy visits per calendar year. This information will be confirmed when we call your insurance company to verify your physical therapy benefits. Physical therapy benefits are also lumped together with chiropractic and occupational therapy. As a result, if you have used any of these other disciplines, your number of physical therapy visits will be affected. Please inform our office staff when making your initial appointment if you have had outpatient physical therapy services in the current year.
  • Do you accept Workers' Compensation?
    Yes. We must obtain pre-authorization prior to scheduling your first appointment. Please provide all Workers Compensation information when your appointment is made.
  • My car accident case is being handled by an attorney. How do I get started at Ho Rehab?
    Ho Rehab Center will accept liens only if an insurance (i.e. car insurance, medical insurance) can be billed for the duration of their physical therapy visits. Ho Rehab DOES NOT accept straight-liens or green liens. The remaining balance following insurance payment will be applied to your lien. Automobile insurance can be billed only if appropriate information is provided at the initial visit and the insurance company accepts liability. Ultimately, the patient is financially responsible for any remaining balances.
